If this is the first time you are accessing the online request form for the Summer 2012 term, once you have selected the course for which you are submitting requests, you are required to read and accept the Alternate Exam Agreement before you can complete your request. If you have already accepted the Alternate Exam Agreement for the current term, you are not required to accept it again.
Have your course outlines available and submit all of the test dates and official exam requests for a course at once.
On the online form, fields marked with an asterisk (*) must be filled in.
SECTION 1: Select a course.
SECTION 2: For the selected course, indicate the number of in-class tests for which you need accommodation.
SECTION 3: For the selected course, indicate the number of official exams for which you need accommodation.
Click Next.
SECTION 4: Review your personal information. Please ensure that the e-mail address you regularly use is the one on the request form and your student record. This is the e-mail address used to notify you of important updates and reminders. Regularly check your JUNK mail folder.
SECTION 5: Fill in the required information for the selected course.
SECTION 6: Fill in the required information regarding the date, time and duration for the number of in-class tests you selected.
SECTION 7: Select the appropriate examination period. Note: For the Summer 2012 term, there are two official examination periods.
S1 term: Wednesday, June 20th to Friday, June 22nd 2012
SU and S2 terms: Tuesday, August 7th to Friday, August 17th 2012
SECTION 8: If applicable, indicate additional notes for the Registrar’s Office.

NOTE: DO NOT use this form if you are requesting to write a test/exam on a different day or time than the class. The online request form can ONLY be used to submit a request if you are writing on the same day and time as the class.
