Work Term Report and Supervisor Evaluation

Work Term Report

When to complete this step: At the end of each work term.

At the end of each work term students are required to submit a Work Term Report, where they reflect on their objectives set in their Learning Agreement and work term experience. There is also an area for students to evaluate their learning and comment on their work performance.

Please use the Work Term Report Template [doc] for developing the report. The report should be saved in .doc or .pdf format only and should be named as follows: CurrentTerm_WTR_YourLastName_YourFirstName. The Work Term Report should be 5 pages double spaced in 12pt Arial font.

Instructions for Students to upload the Internship Work Term Report:

  1. Go to www.yorku.ca/careers/onlinesystem (opens in a new window) and login with Passport York
  2. Click on "profile" in the top navigation bar
  3. Click on the "Internships" tab
  4. Select the record that contains the current term and click the "Edit" button. If you clicked on the wrong work term record or no longer wish to upload a document, click "Cancel" to exit
  5. Go to the "Learning Agreement, Work Term Report & Employer Evaluation (upload)" tab within your record
  6. Click on the "Add Item" button to create a new file upload field for each document you are uploading.
  7. Click the "Choose File" button in the new file upload field you have just created and locate the file you wish to upload. The maximum file size for each uploaded document is 1mb. If possible, please try to keep your upload to 1 document. Please do not submit individual pages as separate documents. The name of your document should be: 'CurrentTerm_WTR_YourLastName_YourFirstName ' (e.g. W12_WTR_Smith_John)
  8. Click "Save"
  9. You will know that your document has been successfully uploaded if you see it listed under "Current Uploaded Reports" at the bottom of the page.

Supervisor Evaluation

When to complete this step: At the end of each work term.

At the end of each work term interns need to submit an evaluation completed by his/her direct supervisor. There are two methods for supervisors to complete the Supervisor Evaluation. It is preferred that supervisors opt for method 1 for consistency and ease of processing.

Please note that it is the student's responsibility to ensure that his/her supervisor completes the evaluation by the deadline.

METHOD 1: Online Form

Supervisors are encouraged to complete the Supervisor Evaluation Form, available through the Career Centre's online system. Benefits for Supervisors choosing to complete the Career Centre's Supervisor Evaluation Form:

  • it's easy to use and can be completed within minutes
  • supervisors can print the form and discuss it with the student prior to submitting it online
  • after the form has been submitted, the student can view it immediately through his/her student profile on the Career Centre's online system

Instructions for Supervisors completing the Career Centre's Supervisor Evaluation Form:

If you are a new user, please contact the Internship Coordinator at ccintern@yorku.ca to request the login information.

  • Login at york-csm.symplicity.com/employers/ (opens in a new window) with the username and password provided to you (please contact the Internship Coordinator if you do not have this login information)
  • Once logged in, go to the “Internship Evaluations” section
  • Find the record with the correct student name and current term in the list. To narrow down your search, you may wish to use the following search filters located at the top of the list:
    • Use the “Term” field to show only records for the applicable term
    • AND/OR type the student’s first or last name in the Keywords field
  • Click on the “Evaluate Student” button beside the correct record
  • Fill in the required information and click "Submit"
  • You may save the form as a draft by clicking the “Save As Draft” button if you wish to finish filling in the form at another time and/or review with the student prior to submitting

Instructions for Students to view their supervisor's completed Supervisor Evaluation Form:

  1. Go to www.yorku.ca/careers/onlinesystem (opens in a new window) and login with Passport York
  2. Click on “profile” in the top navigation bar
  3. Click on the “Internships” tab
  4. Select the record for the current term under "Term". If you clicked on the wrong work term record click "Cancel" to exit.
  5. Go to the “Employer Evaluation (submitted by Supervisor)“ tab within your record
  6. Review and discuss with your supervisor prior to the deadline

METHOD 2: Organization's own Evaluation Form

Supervisors may prefer to complete and submit their own organization’s evaluation form.

  • The student should request a completed and signed evaluation from his/her direct supervisor
  • The student scans the hard copy of the completed evaluation, converts it into a pdf document and uploads it onto the Career Centre's online system (opens in a new window) by the deadline
  • Please note that hard copies will not be accepted. Please do not fax or email the evaluation form.

Instructions for Students to upload supervisor's own organization's evaluation form:

  1. Go to www.yorku.ca/careers/onlinesystem (opens in a new window) and login with Passport York
  2. Click on “profile” in the top navigation bar
  3. Click on the “Internships” tab
  4. Select the record that contains the current term date under "Term" and click the "Edit" button. If you clicked on the wrong work term record or no longer wish to upload a document, click "Cancel" to exit
  5. Go to the “Learning Agreement, Work Term Report & Employer Evaluation (upload)” tab within your record
  6. IMPORTANT: Click on the "Add New" button to create a new file upload field for each document you are uploading (if you do not click on Add New and instead upload a file into one of the existing file upload fields, any documents you have previously uploaded on that record will be overwritten).
  7. Click the "Browse" button in the new file upload field you have just created and locate the file you wish to upload. The maximum file size for each uploaded document is 1mb. If possible, please try to keep your upload to 1 document. Please do not submit individual pages as separate documents. The name of your document should be: 'Current Term_Eval_Your Last and First Name' (e.g. W10_Eval_Smith John)
  8. Click "Save"
  9. You will know that your document has been successfully uploaded if you see it listed under “Current Uploaded Reports” at the bottom of the page.
