3. 2 letters of recommendation from clinical or academic supervisors
4. A cover letter, which should include a description of what the student hopes to achieve from a practicum at Toronto Rehab, and short and long-term career goals
5. Completed Test Administration Form detailing what tests the student has administered, scored, interpreted and written reports for
