What is the purpose of the WSIB Functional Abilities Form?
The WSIB Functional Abilities Form (FAF) is designed to help workers and employers meet their return-to-work obligations. The FAF is completed by the treating practitioner and used to facilitate an early and safe return to work based on discussions between the employer and the ill or injured worker. |

Under what circumstances and when do I complete the Supervisor’s Accident Investigation Report (SAIR)?
The SAIR must be completed under the following circumstances: after an incident occurs, after an employee receives first aid, after an employee seeks healthcare, and after an employee incurs lost time from work. The form must be completed and forwarded to the Employee Well Being Office within 24 hours. |

Why does the department have to pay for the first day of the absence when lost time occurs due to a workplace illness or injury?
Under the Workplace Safety Insurance Act (WSIA), the employer must pay the worker’s wages and employment benefits for the day of the injury. Entitlement to loss of earnings (LOE) benefits starts from the next working day after the day of the injury. |

I am a Faculty Dean/Chair/Manager/Supervisor and the employee/faculty member has been on sick leave and will most likely be on sick leave for 10 days or more. What steps do I take next?
- Request a medical note/certificate to substantiate the absence;
- Complete the Employee/Faculty Member Absence Report (EAR) following 10 days of absence;
- Forward the completed EAR and the medical note/certificate to the EWO.

What if the employee is on sick leave but has no sick credits?
- Request written consent from the employee to apply any remaining vacation and/or personal credits;
- Complete an Employee Transaction Form (ETF) and indicate that a Record of Employment (ROE) be issued to the employee. Then forward the ETF to HR-Records/Payroll and a copy to the EWO. The ROE will assist the employee in applying for Employment Insurance Benefits once they have exhausted all sick credits.

What is the Practitioner’s Report on Abilities and Limitations (PRAL)?
The PRAL assists the Employee Well Being Office in obtaining information from the treating practitioner. It provides:
- The employee’s abilities and limitations, the expected duration of the employee’s absence, and the employee’s ability to return to work with or without accommodation;
- The employee’s consent for the EWO to communicate with the treating practitioner when required. The functional abilities and limitations can be shared with the Manager in order to facilitate a return to work with or without accommodation.

WSIB sent me a Form 6. What is the purpose of this form and why was it sent to me?
The Form 6 is “The Worker’s Report”. It provides the WSIB with details of what happened to cause the injury or illness and with the necessary information required to impartially adjudicate your claim. |

WSIB pays 85% of your net wage. |

Why do I have to fill out the WSIB Functional Abilities Form (FAF)?
Your treating health care professional fills out the FAF by providing information about your functional abilities and how long you can perform the limited activities. By signing page 2, section B of the FAF, you are authorizing your health care professional to provide your employer and the WSIB with this information. |

I am a faculty member/employee and have been on sick leave. What steps do I take next?
- Communicate your absence, according to the provisions of the applicable Collective Agreement or CPM Standard Operating Procedure (SOP), to your Manager/Supervisor/Faculty Dean/Chair. The Home Department will contact the EWO after a ten (10) day absence;
- The Employee Well Being Office will then contact you;
- If the absence continues for more than four (4) weeks the Employee Well Being Office will mail out a Long Term Disability (LTD) application package to you.

What are my options if I require sick leave but do not have any sick credits remaining?
- You can request to use any remaining vacation and or personal credits by contacting your home department
- Apply for Employment Insurance Benefits (EI) upon receipt of a Record of Employment (ROE) from HR-Records/Payroll
