Welcome.

This website houses information from three areas of patient safety research co-led by York University researchers (Ginsburg, Chuang, Tregunno) with researchers from the University of Toronto (Berta) and the University of Calgary (Norton, Flemons) and elsewhere. On these pages you will find descriptions, questionnaire, reports, symposium slides and publication lists from our work in three areas: (1) Patient Safety Culture Measurement, (2) Learning from Patient Safety Failure Events, and (3) early work on the development of a questionnaire to measure Health Professional Education in the area of patient safety. The work described on this site has received funding from several sources including the Canadian Institutes of Health Research (CIHR), the Canadian Patient Safety Institute (CPSI) and York University. Although we have completed studies in each of these three areas, we continue to explore related research questions in these programmatic research areas.

If you would like additional information pertaining to any of this research please contact us here. As well, if your organization would be interested in collecting data using our instruments on learning from patient safety events (The Patient Safety Learning Checklist) or patient safety culture, please contact us.

Finally, we would like to thank the more than 60 Ontario hospitals and numerous health regions from across Canada that have participated in and supported this research.
