Web Questionnaire – Online Consent form


This online questionnaire is divided into two parts. The first part explores student opinions of the
laboratory exercises and student satisfaction with labs in BIOL2070. The second part assesses the
effectiveness of your BIOL2070 TA.

We will be using some of the survey information in a study we are conducting. Below is the information regarding this study.

Study Title: Evaluating and Improving a New Teaching Initiative

Principal Investigators: Paula Wilson, Tamara Kelly, Yi Sheng, Vivian Saridakis, Department of Biology, York University.

Overview/purpose of study: The purpose of this research is to examine the effects of a new second year lab course in Biology. This questionnaire will explore student opinions of the laboratory exercises and student satisfaction in BIOL2070.

Risks/benefits: The study involves no physical or mental risks to you as a participant. You will receive no direct benefit from the study. Indirectly, the research will benefit other students, and future students at York University, by helping us to determine satisfaction and ways to improve the student laboratory experience.

Participation in this study is voluntary; refusal to participate will involve no penalty. Each participant is free to withdraw consent and discontinue participation in this project at any time without prejudice.

Confidentiality: Your responses to the questions in this questionnaire will be kept confidential and anonymous. Data will only be reported in aggregate; no individual responses will be reported.
