1. Web Questionnaire – Online Consent form

Principal Investigators: Paula Wilson, Tamara Kelly, Department of Biology, York University.

Overview/purpose of survey: The purpose of this research is to examine the impact of technology-enhanced courses in Biology.
This questionnaire will explore student opinions of the online resources and classroom technology in BIOL1001. We wish to use
the information to improve the student learning experience.

Risks/benefits: The study involves no physical or mental risks to you as a participant. You will receive no direct benefit from the
study. Indirectly, the research will benefit other students, and future students at York University, by helping us to determine
satisfaction and ways to improve the student laboratory experience.

Participation in this survey is voluntary; refusal to participate will involve no penalty. Each participant is free to withdraw consent
and discontinue participation in this survey at any time without prejudice.

Confidentiality: Your responses to the questions in this questionnaire will be kept confidential and anonymous.
Data will only be reported in aggregate; no individual responses will be reported.
