1. * Informed Consent for Online Survey

Study name: Student engagement in higher education classes

Researcher: Dr. Mara Reich, Faculty of Education, reichm@yorku.ca

Purpose of the research: To examine how student engagement in class is affected by teaching styles/strategies

What you will be asked to do in the research: You will be invited to complete an online survey. There will be multiple choice questions and short answer questions. It should take approximately 15 to 20
minutes to complete the survey.

Risks and discomforts: No foreseeable risks or discomfort from your participation in the research.

Benefits of the research and benefits to you: Useful knowledge about your level of engagement and possible application in your own professional practice as a future teacher.

Voluntary participation: Your participation in the study is completely voluntary and you may choose to stop participating at any time. Your decision not to volunteer will not influence your grades in any
current or future courses, the nature of the ongoing relationship you may have with the researcher, or the nature of your relationship with York University neither now, nor in the future.

Withdrawal from the study: You can stop participating in the study at any time, for any reason. Your decision to stop participating, or to refuse to answer questions, will not affect your relationship with
the researchers, York University, or any other group associated with this project. In the event that you withdraw from the study, all associated data collected will be immediately destroyed wherever
possible.

Confidentiality: All information you supply during the research will be held in confidence and unless you specifically indicate your consent, your name will not appear in any report or publication of the
research. Data will be collected digitally and will be stored in a password-protected file on a password-protected computer. Your data will be safely stored in a locked facility and only the researcher
will have access to this information. Data will be stored on the password-protected computer for two years following completion of the study, at which time all files will be permanently deleted.
Confidentiality will be provided to the fullest extent possible by law.

The researcher acknowledges that the host of the online survey may automatically collect participant data without their knowledge (i.e., IP addresses). Although this information may be provided or
made accessible to the researcher, it will not be used or saved without participant’s consent on the researcher’s system. Further, because this project employs e-based collection techniques, data may
be subject to access by third parties as a result of various security legislation now in place in many countries and thus the confidentiality and privacy of data cannot be guaranteed during web-based
transmission.

received ethics review and approval by the Human Participants Review Sub-Committee, York University’s Ethics Review Board and conforms to the standards of the Canadian Tri-Council Research
