Peak periods may experience delays.<\/li><\/ul>\n\n\n\n<p><\/p>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<p><strong>Requirements<\/strong><br><br>Test Scoring OMR forms brought in for processing must fulfill the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Forms must have a completed and printed <a href=\"https:\/\/www.yorku.ca\/printing\/wp-content\/uploads\/sites\/52\/2021\/11\/Test-Scoring_022613.doc\" data-type=\"URL\" data-id=\"https:\/\/www.yorku.ca\/printing\/wp-content\/uploads\/sites\/52\/2021\/11\/Test-Scoring_022613.doc\">cover sheet.<\/a><\/li><li>Forms must be secured by elastic bands or placed in envelope.<\/li><li>Forms cannot be crumpled or folded.<\/li><li>Forms must be organized and all facing in the same order.<\/li><li>Forms must have an unmarked timing line. <\/li><\/ul>\n\n\n\n<p><\/p>\n<\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<p><strong>Access<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>York faculty, Instructors and Administrative Staff can order Test Scoring OMR forms, by filling out, printing and returning the completed&nbsp;Test Score and Course Evaluations OMR Order Form&nbsp;on our Resources page.<\/li><li>To request processing of completed Test Scoring OMR forms, please bring them to 159 Central Square during office hours with the&nbsp;cover sheet&nbsp;printed. After hours drop-off is also available through mail slot at 159 Central Square.
