{"id":33280,"date":"2017-03-21T04:00:00","date_gmt":"2017-03-21T08:00:00","guid":{"rendered":"https:\/\/www.yorku.ca\/researchdev\/2017\/03\/21\/york-u-leads-research-on-tool-to-assess-hospital-patient-readmission-risk-2\/"},"modified":"2025-01-30T08:49:52","modified_gmt":"2025-01-30T13:49:52","slug":"york-u-leads-research-on-tool-to-assess-hospital-patient-readmission-risk-2","status":"publish","type":"post","link":"https:\/\/www.yorku.ca\/research\/2017\/03\/21\/york-u-leads-research-on-tool-to-assess-hospital-patient-readmission-risk-2\/","title":{"rendered":"York U leads research on tool to assess hospital patient readmission risk"},"content":{"rendered":"<div id=\"wrapper\" class=\"hfeed\">\n<div id=\"wrap\" class=\"container clearfix\">\n<section id=\"content\" class=\"primary\">\n<article id=\"post-240336\" class=\"post-240336 post type-post status-publish format-standard has-post-thumbnail hentry category-top-stories tag-community tag-health tag-research tag-top-stories\">\n<div class=\"entry clearfix\">\n<p>A team of researchers led by York University <a href=\"http:\/\/health.yorku.ca\/health-profiles\/index.php?mid=981753\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Professor Christo El Morr<\/strong><\/a> has published its findings on the success of using a specific real-time tool to assess the risk of unplanned readmission of hospital patients.<\/p>\n<div id=\"attachment_237203\" class=\"wp-caption alignright\" style=\"width: 215px\">\n<p><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-237203\" src=\"http:\/\/yfile.news.yorku.ca\/files\/2017\/01\/ChristoElMorrSCOOP.jpg\" alt=\"\" width=\"205\" height=\"205\" \/><\/p>\n<p class=\"wp-caption-text\">Christo El Morr<\/p>\n<\/div>\n<p>The two-year collaborative project was funded by North York General Hospital Exploration Fund, and the team included Professor Liane Ginsburg (Co-PI) from York University, York U School of Health Policy &amp; Management undergraduate student Seungree Nam, and Susan Wollard, director of the Medical, Critical Care &amp; Elderly Care Program at North York General Hospital.<\/p>\n<p>The study, \u201cAssessing the Performance of a Modified LACE Index (LACE-rt) to Predict Unplanned Readmission After Discharge in a Community Teaching Hospital,\u201d was published in\u00a0JMIR Publications here:\u00a0<a href=\"http:\/\/www.i-jmr.org\/2017\/1\/e2\/\">i-jmr.org\/2017\/1\/e2<\/a>.<\/p>\n<div id=\"attachment_240082\" class=\"wp-caption alignleft\">\n<p><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-240082\" src=\"http:\/\/yfile.news.yorku.ca\/files\/2017\/03\/liane-ginsburg.jpg\" alt=\"\" width=\"166\" height=\"204\" \/><\/p>\n<p class=\"wp-caption-text\">Liane Ginsburg<\/p>\n<\/div>\n<p>The LACE index is used to predict unplanned readmission of patients, or patient death, after discharge, and considers the length of stay; the acuity of admission; co-morbidities; and the number of emergency department visits within the past six months. However, until now, its implementation and use on a daily basis in real-life situations were never assessed. The team adapted LACE for implementation in a hospital setting; the adapted LACE was used in real time, hence the name LACE-rt, said Ginsburg.<\/p>\n<p>\u201cOur team assessed LACE-rt and found it a fair tool for identifying patients at high risk of readmission,\u201d said El Morr. \u201cWe found high-risk patients are 2.648 times more likely to be readmitted than those at low risk. We have also identified data entry errors caused by a lack of information system integration.\u201d<\/p>\n<p>The team, he said, has already designed an updated version of the software interface that minimizes data entry and avoids errors, and has developed plans to assess interventions targeting readmission reduction.<\/p>\n<div id=\"attachment_240516\" class=\"wp-caption alignright\" style=\"width: 189px\">\n<p><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-240516\" src=\"http:\/\/yfile.news.yorku.ca\/files\/2017\/03\/Susan-Woollard.jpg\" alt=\"\" width=\"179\" height=\"202\" \/><\/p>\n<p class=\"wp-caption-text\">Susan Woollard<\/p>\n<\/div>\n<p>\u201cThe project highlights the need for collaborative efforts that bridge acute and community care sectors,\u201d said Ginsburg.<\/p>\n<p>Assessing readmission risk and reducing hospital readmission to acute care is of paramount importance for patient health, quality of care and cost reduction, said El Morr.<\/p>\n<p>\u201cThe collaborative work between researchers in the School of Health Policy &amp; Management and collaborators at North York General Hospital has culminated in a new understanding of the performance of LACE index in a real-life environment,\u201d said El Morr. \u201cThe collaboration with North York General Hospital was an excellent experience; we encountered positive leadership and supportive environment. It allowed Prof. Ginsburg and myself to engage our students in experiential learning and produce research that has impact.\u201d<\/p>\n<p>Wollard said of the project: \u201cI think this is a tremendous work that will support our discharge planning in the future.
