The Modified Stanford Instrument (MSI) is a Patient Safety Culture questionnaire
that has been used to measure staff perceptions of patient safety culture
in healthcare organizations:
- THE CAN-PSCS (FORMERLY THE MSI PATIENT SAFETY CULTURE SURVEY)
The 2-page 2010 version of the survey is presently being used in a number of Canadian jurisdictions with multiple staff groups and different care settings. Beginning in 2014, the CAN-PSCS will be implemented as part of the Canadian Accreditation Process.

- RESEARCH ON MEASURING PATIENT SAFETY CULTURE
The properties of the survey continue to be assessed in a large study
funded by the Canadian Patient Safety Institute. A summary of the study
can be found on the CPSI's

- IMPLEMENTATION HANDBOOK
The survey is being used for research as well as monitoring and improvement
activity within healthcare organizations. As part of this study we devised
an implementation handbook (PDF)
for organizations who are using the survey.

We have been working on ways to help organizations interpret the findings.
A sample lay report with a summary of findings, some comparative benchmarking
data, and recommendations has been provided to some organizations who
