Non-Occupational Absence and Accommodation
A completed Medical Absence/Accommodation Certificate (MAAC) must be provided to the Employee Well-being Office via ewb@yorku.ca to clarify abilities, restrictions, limitations and duration of absence. Policies, procedures, and forms pertaining to non-occupational absence, leave, and accommodation can be found on YU Link. Note that you will be required to login using your Passport York information to access this website.
Family Status Accommodation
A completed Request for Accommodation - Family Status must be submitted to Employee Well-Being via FAMSACC@yorku.ca advising of the reason for the request and detailing the Family Status responsibilities. Details regarding the process and forms pertaining to family status accommodation can be found on YU Link.
