The following documentation must be completed by Teacher Candidates and submitted to the Practicum Facilitator using the Mach Form link provided prior to the start of the Practicum placement. Teacher Candidates must arrange to meet with their Mentor Teacher to complete paperwork prior to the practicum start date. All forms are available in the TLSE 3715 6.00 course in eClass.
- Present a Criminal Background Check including a Vulnerable Sector Screening (VSS) to the Board/School as per Board regulations.
- View the Ontario Ministry of Labour Health and Safety at Work e-Learning Video and upload a copy of the "Proof of Completion Certificate”
- Print, sign, and upload: WSIB Student Declaration of Understanding (WSIB/Private Insurance Coverage)
- Part-time Teacher Candidates who have received approval from the Practicum Facilitator to use their current place of employment for one practicum placement must complete a WSIB Placement Acknowledgement. This Acknowledgement must be signed by a school board Administrator with signing authority at your school. This document is to acknowledge that York University will not provide any insurance coverage to the Teacher Candidate during their practicum placement days. The completed form must be returned to the Practicum Facilitator before the practicum placement commences.
- Identification Badges: Teacher Candidates are expected to have the digital YU-Card with them at all times during placement and will be expected to present this Photo ID upon request during placement.
Teacher Candidates are not allowed to be in any school or Practicum-related setting until the above forms have been completed and submitted to the Practicum Facilitator.
