MSH consent form

This form provides your consent for the use of your recorded interview/video by the Mad Studies Hub at York University for publication and promotional purposes.

Consent and Release Agreement

I hereby grant permission to the Mad Studies Hub at York University to publish, distribute, reproduce, and publicly display my submitted video across its official platforms and communication channels. These may include, but are not limited to, the Hub’s website, social media platforms (such as YouTube), digital newsletters, public events, academic presentations, and other educational or outreach materials.

I understand that this permission is granted voluntarily and without expectation of compensation. I waive any claims related to payment, royalties, or financial benefit resulting from the use of the video.

Permission to Publish

I authorize the Mad Studies Hub to share, reproduce, display, and distribute my submitted video for academic, educational, research, advocacy, community engagement, and promotional purposes.

Waiver of Rights and Claims

I understand that once the video is published, it may be publicly accessible. I waive any right to review or approve future uses and release the Mad Studies Hub and York University from any related claims.

Intended Use

The video may be used across various digital and print formats aligned with the Hub’s objectives. Any use outside this scope will require additional written consent.

Ownership and Third-Party Rights

I confirm that I am the original creator of the submitted video or have obtained all necessary permissions for any third-party content included. I confirm that the video does not infringe on any legal rights.

Revocation of Consent

I understand that I may request withdrawal of this permission by submitting a written request to madstudieshub@yorku.ca. However, removal of already published content cannot be guaranteed.
