MachForm is an online system for creating fillable forms and managing the data you collect with them. MachForm is well-suited for collecting data via application or registration forms.
There is no centralized access, but rather many installations across the University. This training process is for the LA&PS staff and faculty only. Please refer to your unit for more guidance.
Online Training
An online self-paced training course for MachForm is available for faculty and staff on YU Learn titled "Forms Learning Package"
After completing this training, please contact esohelp@yorku.ca for MachForm access or for any further inquiries.
Topics Covered:
- Create, duplicate and edit forms
- Check and download entries
- Manage e-mail notifications
- Add tags to forms
- Understand additional features available through MachForm (spam protection, website integration etc.)
Security Measures
Ensure that all previously created and new forms follow the requirements for CAPTCHA Spam protection and Departmental Tagging. As part of regular maintenance, forms will be reviewed to confirm that CAPTCHA and security settings are in place.
Please make sure your forms are updated, as accounts without these settings may be temporarily suspended.
